Public Health Trust of Miami-Dade County (PHT) / Jackson Health System
SFCCN provides services to Medicaid families in Miami-Dade and Broward Counties.
SFCCN works with the Children's Medical Services Network (CMSN) to provide services for CMS enrolled Medicaid children and their siblings in Broward County and Florida KidCare enrolled children in Miami-Dade, Monroe, Broward, Palm Beach, Martin, St. Lucie, Indian River and Okeechobee Counties. The CMSN serves children with special health care needs who are Medicaid or Florida KidCare eligible.
