Provider Services
South Florida Community Care Network (SFCCN) is a partnership fully owned and operated by South Florida's most experienced names in healthcare: Broward Health/North Broward Hospital District (NBHD) and Memorial Healthcare System (MHS) in Broward and Public Health Trust (PHT)/Jackson Health System in Miami-Dade.
SFCCN, through a contract with the Agency for Health Care Administration, provides services to Medicaid populations in Broward (Reform) and Miami-Dade (non-Reform) counties as a Provider Services Network (PSN). As the first PSN in Florida, SFCCN has provided Medicaid services to two of the largest counties in the state since March 2000.
SFCCN has also partnered with the Department of Health, Children's Medical Services (CMS) to form an Integrated Care System (ICS) in Broward, Miami-Dade, Monroe, Palm Beach, Martin, St. Lucie, Okeechobee and Indian River Counties. The ICS is known as the Children's Medical Services Network South Florida Region (CMSN-S) in Dade and Monroe and the Children's Medical Service Network Southeast Florida Region (CMSN-SE) in Broward, Palm Beach, Martin, St. Lucie, Okeechobee and Indian River. The CMSN serves Florida KidCare children with special health care needs in all of these counties and Medicaid children in Broward County only. The CMSN-Broward is a Medicaid Reform PSN in Broward County serving children with special healthcare needs and their siblings.
NBHD, MHS, and PHT are subnetworks of SFCCN. Each subnetwork has a Provider Relations Department that works closely with its participating providers to ensure that Medicaid families and CMS children in the South Florida area receive quality health care and services. Each department is available to assist providers and their offices with the procedures required by SFCCN and CMSN which would include (but is not limited to):
- new provider orientation
- assistance with reporting requirements
- educational overviews on compliance issues
- on-site support
- assistance with address and other practice changes, and
- questions regarding procedures, policies, reimbursement, and other program information
Provider Relations Representatives from each Provider Relations Department conduct routine visits of SFCCN and CMSN provider sites. During these visits, the Provider Relations Representative assesses the practice's total compliance with various regulatory and program standards, including:
- access to care
- physical accessibility to the practice environment
- medical record keeping practices
- patient confidentiality procedures
- physical appearance and adequacy of the facility
- appropriate staffing (medical and administrative)
- OHSA compliance
- grievance procedures, and
- peer review procedures
Provider Relations Department Representatives are available to assist you with any of the services outlined above from 8:00 am to 7:00 pm Monday through Friday.
