What is SFCCN?
South Florida Community Care Network is South Florida's three major hospital districts – Broward Health, Memorial Healthcare System and Jackson Health System – working in partnership as a provider service network. SFCCN offers services from child health checkups to surgery, home health to laboratory tests. As the premier community hospitals serving South Florida, the SFCCN partners provide quality care for you and your family.

How do I enroll in the SFCCN?
Click on the SFCCN Enrollee Information Tab at the top of this page. In this section, you will find information about eligibility and enrollment.

If my child is in SFCCN CMSN, is he/she still on CMS?
Yes, SFCCN CMSN is a contracted program within CMS.

Can I use SFCCN outside of Florida?
Emergency services are covered out of state. Out of state referrals for non-emergency services can occur when there is a medical need and services cannot be provided within the State of Florida. Prior authorization for non-emergency out of state services will need to be obtained.

In what areas does SFCCN serve Medicaid?
Miami-Dade County (PHT) and Broward County (NBHD, MHS)

What is a Primary Care Provider (PCP)?
A primary care provider is a doctor that manages most of your health care needs by working with specialists who care for you. You get all your well care and non-emergency care from your PCP. You should call your PCP any time you need non-emergent care.

What is a Medical Home?
CMS believes that every child should have a medical home that is compassionate and culturally effective. Your medical home provides primary health care services. It provides comprehensive, coordinated, on-going family-centered care.

How do I change my PCP?
Call the Enrollee Services phone number on your ID card.

How do I know which doctor(s) I can go to?
Go to the Provider Directory Tab. Click on the card that looks like yours. There you will see the doctors in your network. The provider directory lists all providers that are participating in your plan. You can also call the Enrollee Services phone number on your ID card and request a copy of the Provider Directory.

I need to see a specialist. How do I find one?
Specialists are listed in the provider directory. Work with your PCP to locate the appropriate specialist and obtain a PCP referral to that specialist. If you need assistance, call the Enrollee Services phone number on your ID card.

The network does not have the provider I need. Can I go to a provider not on the list?
Yes, if it is medically necessary and has been authorized by SFCCN or CMSN. Your PCP will need to get the authorization.

Do I have to pay for services?
The only cost to you for approved services is the Medicaid co-payment and/or co-insurance, if applicable. A co-insurance is part of the cost of services. You will need to pay for services the SFCCN doesn't cover. The doctor or facility must tell you if you need to pay. If you do receive a bill by mistake, DO NOT ignore it. Please call the Enrollee Services phone number on your ID card.

What is an Advance Directive?
You can decide now what medical and mental health care you do and do not want if you get very sick. You can ask not to have certain help. You can also ask not to be kept alive with special care. If you are 18 years of age or older you can write your wishes. This is called a living will. You can tell your doctor what kind of care you want or don't want. You can also name someone to make healthcare choices for you. You should speak to your doctor for help on this. Complaints on this can be filed with the State's Complaint Hotline at 888-419-3456.

Why do I need to complete the Health Needs Questionnaire?
The Health Needs Questionnaire is the green paper that comes in your new enrollee packet. It is important that you fill out this form and return it in the self-addressed envelope provided. NO POSTAGE IS NEEDED. This form helps us to identify your medical needs. A completed form will let us help you get the medical services you need.
